Evidence-based systematic review: effects of oral motor interventions on feeding and swallowing in preterm infants.

چکیده
PURPOSE To conduct an evidence-based systematic review and provide an estimate of the effects of oral motor interventions (OMIs) on feeding/swallowing outcomes (both physiological and functional) and pulmonary health in preterm infants. METHOD A systematic search of the literature published from 1960 to 2007 was conducted. Articles meeting the selection criteria were appraised by 2 reviewers and vetted by a 3rd for methodological quality. RESULTS Twelve studies were included and focused on 3 OMIs-nonnutritive sucking (NNS), oral/perioral stimulation, and NNS plus oral/perioral stimulation. Six studies addressed the effects of OMI on the feeding/swallowing physiology outcomes of feeding efficiency or sucking pressures. Ten studies addressed the functional feeding/swallowing outcomes of oral feeding or weight gain/growth. No studies reported data on pulmonary health. Methodological quality varied greatly. NNS alone and with oral/perioral stimulation showed strong positive findings for improvement in some feeding/swallowing physiology variables and for reducing transition time to oral feeding. Prefeeding stimulation showed equivocal results across the targeted outcomes. None of the OMIs provided consistent positive results on weight gain/growth. CONCLUSIONS Although some OMIs show promise for enhancing feeding/swallowing in preterm infants, methodological limitations and variations in results across studies warrant careful consideration of their clinical use.

A Review of Assessment Tools for Oral Feeding in Term and Preterm Infants
Background and purpose: Problems in oral feeding are common in infants, especially in preterm infants. Therefore, assessment of infants feeding could be of great help in making appropriate diagnosis for rehabilitation of swallowing. Therefore, comprehensive and accurate assessment tools are needed. The aim of this study was to review assessment tools used for infants oral feeding. Materials an...
متن کاملThe Effect of Non-Nutritive Sucking on Weight Gain and Length of stay in Preterm Infants
Background: Preterm infants frequently experience oral feeding difficulties due to undeveloped oral- motor skills and lack of coordination between sucking, swallowing and breathing. Infants’ ability to orally consume all feedings while maintaining physiological stability and weight gain is necessary for their discharge. Aim: The effect of non-nutritive sucking on weight gain in preterm infants....
متن کاملEffect of Body Position on Physiological Alteration While Feeding Infants: A Systematic Review
Background: An infant should be able to coordinate sucking, swallowing, and breathing to have a safe and effective feeding, which is developed with the increasing gestational age. Any problem in each of these functions can lead to the risk of aspiration, pneumonia, decreased oxygenation saturation, apnea, and bradycardia. The changed body positions may cause changes in the physiological efficie...
متن کاملThe Effect of Oral Motor Intervention on Oral Feeding Readiness and Feeding Progression in Preterm Infants
Background: Advances in neonatal intensive care have markedly improved survival in preterm infants. These babies need hospitalization due to the immaturity of the respiratory and digestive systems. The timing for the initiation of proper feeding in premature infants admitted to neonatal intensive care units (NICUs) is an important challenge for physicians, parents, and...
متن کاملThe Effect of the 5 Minute Premature Infant Oral Motor Intervention (PIOMI) Versus the 15 Minute Oral Stimulation Program for Breastfeeding Achievement in Term Infants with Feeding Problems: A Randomized Clinical Trial
Objectives: Feeding problems can occur in early-term infants born at 37 weeks; Early-term and term infants may benefit from oral motor therapy to attain successful breastfeeding. The present study aimed to determine the impact of the 5-minute premature infant oral motor intervention (PIOMI) and the 15-minute oral stimulation program (15 minute Oral Stimulation Program) on 37-41 week infants in ...
